Death as a result to warping and /fly off

Bug:I have died two times in the last two days when warping into the wild spawn. The death was I assume a result from taking fall damage when loading into the server. Also while mining in the wild I typed /fly to turn off fly and died instantly. I was wearing prot IV armor each time when this happened as well as carrying god picks. I understand that this may just happen intermittently but replacing all that gear can become quite costly and time consuming.
Time/Date:2/3/14 and 2/4/14
Steps to reproduce: I have not tried to purposely recreate this but I will try with no gear on later tonight if it is desired.
Evidence:No evidence as of yet because all you would see is a death screen.

If you leave /fly on and warp somewhere, when you return to that world /fly is off and you fall.

Also, I have found that fall damage is extremely heightened when you turn off /fly; you can often die from just falling a few blocks.

I've remedied these issues myself by trying to remember to either always turn /fly off before leaving a world, and being safely on the ground when doing so, or carrying little-no items when travelling between worlds.

 

yeah I knew about the /fly thing when warping and I am almost positive it was off when I warped. One of the times I warped from skyblock to wild and I think it was the first time I had been in the wild on that specific log in. The time I turned of fly I was literally less than one block high and died instantly, I know this because I purposefully flew down to the ground prior to turning it off for that very reason. I know this is an issue that may not be able to be fixed but I wanted to bring it to yall's attention anyway. I will be more careful in the future but if you guys can fix this it would be much appreciated.
